Great comments Frances. I agree. There are inequities as well as situations that make it difficult to control quality. Monitoring is the key. Volunteers can play a role in reporting on quality.
I have seen the same with HIPAA...enforcement of the law is weak. Every once in a while they fine an organization for poor practices and it raises awareness but in recent years it has been quiet once again.
