  • Here are a few of the IDG notes that I used for my patients' plan of care: 

    • Initial Volunteer Assessment
      • Example: "IDG 1/6/22 - Initial Volunteer Assessment - Patient/family decline need for hospice volunteer services at this time but remain open to possibility of utilizing the service in the future. Will continue to follow for any developing volunteer needs."
    • Weekly/Biweekly notes on patients utilizing our volunteer services
      • Example: "IDG 1/6/22 - Patient continues to receive weekly caregiver respite visits with hospice volunteer John Doe. Will continue to follow for any additional volunteer needs."
    • Recertification notes
      • Example: "IDG 1/6/22 - Recertification - Patient/family continue to decline need for hospice volunteer services. No new needs identified by IDG team. Will continue to follow for any additional volunteer needs."
