I am new to my position and we are reviewing forms we have been using as we struggle to build our pool of volunteers. We want to make sure we capture the best, most relevant info during the volunteer visit with the patient. Does anyone have a form (I think we call ours a "Volunteer Contact Record") that they'd be willing to share for me to get some ideas? Thanks for any help!

  • Unfortunately, we are still paper and pencil here. I only have limited access to the computer and the nurses were lamenting the lack of computer access earlier today. I think it will be awhile before we all go online. I did pull the samples from the document index. I was just looking for additional samples to compare to. (I like to overwhelm myself sometimes!) Thanks!
  • The Hospice Volunteer Association's (HVA) new Patient Data Vault Service (PDV) has a whole series of hospice volunteer management forms that are automatically provided with the service. The PDV is a low-cost solution for managing and communicating with volunteers online while meeting the strict requirements of the HIPAA Privacy Rule.

    There are still a limited number of openings for hospices interested in participating in the pilot program, which is ongoing. All services are free during the 2-4 month pilot program period and participants have the option of opting out of the service at the end of the pilot period if they are not satisfied with the service.

    Additionally, HVA has a repository of forms, documents and articles on end-of-life care submitted by other volunteer coordinators. HVA created this National Hospice Document Repository (NHDR) as a public service to the hospice community. Access to this repository is free and it is the largest collection of documents and articles relating to hospice and end-of-life care in the world. The articles section has a Google search to easily find the articles with specific topics.
